Obtaining Medical Records or Birth/Death Certificates

How to Obtain a Copy of your Medical Records from Hartford Hospital or Hartford Hospital’s Institute of Living
Hartford Hospital can provide or release copies of medical records at the written request of patients or their authorized legal representatives. (If authorization is signed by a Legal Representative, a copy of the appropriate documentation verifying the individual's authority must be provided with the authorization.) There may be a per-page fee for copies. Follow the steps below:

Requests are processed on a first come, first served basis. Information is mailed to patients -- or they can chose to pick it up at Hartford Hospital, Bliss Building, Room B139, First Floor. (Patients will get a phone call when the copies are ready to pick up.)

How to Obtain Birth and Death Certificates
Hartford Hospital does not retain copies of birth or death certificates. This archive is maintained by the City of Hartford's Bureau of Vital Records.
Hartford Hospital can provide or release copies of medical records at the written request of patients or their legal representatives.
Birth Certificates
A Hartford birth certificate may be obtained in person, through a mailed-in request, or online through VitalChek. Note that in accordance with State of Connecticut Statutes, birth certificates are legally available to:
- The person him/herself sixteen years of age or older
- A parent, spouse, grandparent or child of said person
- A legal representative or legal guardian with appropriate proof of legal status
Death Certificates
A Hartford death certificate may be obtained in person, through a mailed-in request, or online through VitalChek. Mailed-in requests must be accompanied by a check or money order made payable to the City of Hartford. See the City of Hartford website for further details.
